Back Pain People Wait Too Long On: When It Is Worth A Urology Check
Most back pain is musculoskeletal. A smaller group is coming from the kidney — and that group waits far too long before anyone looks.

The short answer
Back or loin pain that keeps returning, especially on one side, with any change in your urine, is worth a urological check rather than another round of painkillers.
What points towards the kidney
- Pain in the loin (side of the back, below the ribs) rather than the low back
- Pain that comes in waves and does not settle with position or rest
- Nausea or vomiting with the pain
- Blood in the urine, burning, or passing urine more often
- Fever with loin pain — this needs to be seen the same day
What points away from it
Pain that is worse on movement, tender to press over the spine, or clearly linked to lifting is usually musculoskeletal. That does not make it trivial; it makes it a different problem.
What the assessment involves
History and examination first. A urine test, and where indicated an ultrasound or a non-contrast CT of the kidneys, ureters and bladder. Each test is ordered for a stated reason, and the reason is explained before it is booked.
Why the wait matters
An obstructed kidney can lose function quietly. Most people who present late did not ignore the pain — they were treating it as a back problem. If the pattern above fits, get it checked.
